IR-004: The Missing Screwdriver

INCIDENT REPORT / IR-004
Stores has received a report concerning one flat-head screwdriver last observed being placed “somewhere safe.” Its present location remains unknown.
All personnel have been questioned. Each confirmed that they did not use it, have not seen it and were not responsible for leaving it wherever it eventually turns up.
The approved corrective action is the immediate purchase of a replacement screwdriver. Previous incidents indicate that this will cause the missing item to reappear in plain sight within approximately forty minutes.
The replacement will then also go missing, providing operational resilience through duplication.
Status: report closed pending inevitable recurrence.
